ONLY instructor signatures are accepted. Check balances are generated from this form and if it is submitted incomplete and/or not submitted by the deadline, this will result in a delay or reduction of your check balance. Second session classes are paid after the 1st day of classes begin. Obtain instructor signatures on the 1st day of class and submit to FAO.
